長者健康評估先導計劃 (EHAPP) 

隨著人口老化及慢性疾病增加，我們需要加強長者的預防性護理工作，本院現與衞生署合作，推出「長者健康評估先導計劃」，為一眾長者進行健康評估。先導計劃旨在識別長者的風險因素、生活模式及其疾病，務求適時和針對性地處理這些風險因素和健康問題。 
本先導計劃是參考《香港長者護理參考概覽─長者在基層醫療的預防護理》而制訂，並由下述三個部分組成： 
(一) 基線健康評估 
(二) 一至兩次的跟進諮詢 
(三) 推廣健康環節 
服務對象：
- 年滿70歲；
- 持有有效香港身分證或豁免登記證明書的長者；
- 並非為衞生署轄下長者健康中心的會員(沒有衛生署紅簿仔)。

Elderly Health Assessment Pilot Programme (EHAPP) 

To facilitate early identification of risk factors and to promote healthy ageing, we have launched the Elderly Health Assessment Pilot Programme (EHAPP) in collaboration with the Department of Health by providing voluntary, protocol-based, subsidized health assessment. The health assessment seeks to identify risk factors (including lifestyle practices) and diseases so that risk factors and health problems identified can be managed in a timely and targeted manner.

Having regard to the “Hong Kong Reference Framework for Preventive Care for Older Adults in Primary Care Settings” developed by the Task Force on Conceptual Model and Preventive Protocols of the Working Group on Primary Care, the Pilot Programme will comprise three components –
(a) baseline health assessment;
(b) follow-up consultations; and
(c) health promotion.
 

Service Target :
- aged 70 or above
- hold a valid Hong Kong Identity Card
- not a member of Elderly Health Centre under the Department of Health
